Library Cards for Students

Bring your classes to visit the Mesa Library!
Ahead of time, download the PDF for the Mesa Library card application, photocopy and distribute the applications to your students, so they can bring their completed and parent/guardian-signed forms the day of the library visit.
Students should have identification (although a completed application with a parent/guardian signature is acceptable without ID for groups), and know their current phone number, address and guardianship information. If you have any questions, please call the Mesa Library at (480) 644-2207.

Library Cards for Minors under 18 years of age
Minors without ID must have a parent or legal guardian present. School groups are the only exception. A child in a group does not need ID if he/she brings in the completed application form signed by his/her parent/legal guardian.
Minors must also know their current phone number, address and guardianship information otherwise a guardian with ID must be present.
 
If a minor has forgotten their card, we will issue a Forgot Cards following the same policies.
A parent or legal guardian must be listed on the minor's library card account and must accept responsibility for all items borrowed. When a minor applies for a library card, a letter is mailed to the parent or guardian explaining their child has been issued a Mesa Public Library card and the responsibilities of having the card.
